We want to make you feel welcome here. Please read through our patient section for all necessary forms, information and what to expect. Any question not answered here, do not hesitate to give us a call.

CLINIC EXPECTATIONS

  • We suggest wearing loose fitting and comfortable clothing during your appointment.
  • Please turn your cell phones off or on vibrate during your appointment times.
  • Please do not wear fragrances, perfumes or heavy scents.
  • You should expect the same results and relief from each therapist.
  • If you bring your child(ren) to the clinic, we ask they stay in the waiting area for safety reasons.
  • Please plan on coming 15-30 minutes prior to your appointment time to fill out paperwork and so we can scan your insurance card, photo identification and referral.

SPECIALITIES

Alternative Back Care Physical Therapy is not your traditional Physical Therapy clinic. We have innovative and revolutionary techniques, specializing in back pain and migraines, as well as treatment for on the job injuries, sports injuries, post operation and post-surgical care, shoulder rehab and other Physical Therapy treatments. We use many different modalities such as the Leveling Technique ®, IASTM, Cupping, heat, ice, E-Stem, ultrasound and soft tissue massage in addition to our unique plan of care, self-alignment techniques and exercises.

MASSAGE THERAPY

Alternative Back Care Physical Therapy also offers Massage Therapy with our licensed massage therapy practitioners. We offer injury massage, myofascial massage, Swedish massage, trigger point massage, Cupping, deep tissue massage, relaxation massage, TMJ or oral massage, Zen Bodytherapy® and sports massage.

Massage Therapy in combination with Physical Therapy can greatly increase the recovery time of injury. Our highly training massage therapists work with your Physical Therapists to create a plan of care to help you have the most successful treatments while here.

WHAT TO EXPECT DURING TREATMENT

Your first visit will be approximately an hour long. Your Physical Therapist will discuss your medical history and perform an evaluation. During your evaluation your therapist will evaluate strength, pain levels, range of motion, general mobility, functional limitations and medical history.

Your Physical Therapist will work with you and your physician to determine a plan of care, a therapy team and the length and frequency of care that is most appropriate for you. Your therapist will remain in close contact with your doctor throughout your treatment via progress notes and phone calls.

Your treatment may include manual therapy, exercises and other treatment modalities such as IASTM, Cupping, EStem, Ice, Heat, Ultrasound and soft tissue work.

At ABC PT, we understand that information about you and your health care is personal. Please view the HIPAA notice of privacy practices to better understand all of your rights.

We do require written authorization from the patient to give out any Personal Health Information ADD FORM HERE.

CANCELLATION POLICY

Please arrive on time for all appointments so you can have a full treatment session. If you are more than 15 minutes late that can count as a no-show appointment. We do require 24 hours notice for cancellations to avoid the $50 cancellation fee for Physical Therapy appointments and $25 cancellation fee for Massage Therapy appointments.

INSURANCE

Depending on your insurance, we require specific items in order to bill your insurance. Please see below to find what we will need in order to bill your insurance. If you are unsure of what is required of your insurance, please contact our office or your insurance company.

Most insurances require us to collect copayments and coinsurances at the time of the visit. For your convenience we accept cash, checks, debit or credit.

At this time we do not accept Amerigroup, Coordinated Care, Kaiser HMO, Humana and Ambetter.
