Do You Need a Doctor's Referral to See a Physical Therapist in Washington? Not Anymore.
Your back went out on a Saturday morning. Or, you twisted your knee at the gym. Or, you've been living with nagging shoulder pain for weeks, dreading the idea of making a doctor's appointment just to get a piece of paper that says you can go see someone who can actually help.
Here's some good news: in Washington State, that piece of paper isn't required.
Thanks to direct access laws, Washington residents can schedule an appointment with a licensed physical therapist — like the team at ABC Physical Therapy — without a physician's referral. You can call us directly, come in, get evaluated, and start treatment. It's that simple.
What Is Direct Access Physical Therapy?
Direct access means exactly what it sounds like: direct access to a physical therapist, without needing to see a doctor first.
Traditionally, many patients assumed they had to visit their primary care physician, get a diagnosis, obtain a written referral, and then make a physical therapy appointment. That model made sense in some situations, but it also created unnecessary delays, extra costs, and more hoops to jump through when you just need to get moving again.
Under direct access, physical therapists are recognized as qualified first-contact providers. We're trained to evaluate your condition, screen for serious issues that may need a physician's attention, and design a treatment plan — all without waiting on a referral.
Washington State's Direct Access Law
Washington is one of the states that allows direct patient access to physical therapy, with a couple of specific provisions in place:
Foot and ankle orthotics: A physical therapist may only provide orthotic treatment for foot or ankle conditions by referral or consultation from an authorized healthcare provider.
Beyond-scope conditions: If your symptoms or condition fall outside what a physical therapist is trained to treat, your PT is required to refer you to an appropriate physician.
Outside of these situations, you are free to seek physical therapy evaluation and treatment directly — no prescription, no referral needed.
Does Insurance Cover Direct Access PT?
This is the question most patients ask, and the answer is generally yes — though it depends on your plan.
Most major insurance plans in Washington, including Medicare, Medicaid (Apple Health), and commercial carriers, do not require a physician referral for physical therapy services. They align with Washington state law and will cover your visits directly.

For patients who prefer a simpler path, we also accept self-pay, which lets you skip the insurance process entirely.

When Should You Still See a Doctor First?

You're experiencing chest pain, difficulty breathing, or other symptoms that could signal a serious medical condition
You've had significant trauma — a car accident, a fall from height, or a severe injury
You suspect a fracture or dislocation
You have unexplained neurological symptoms like sudden numbness, weakness, or loss of bladder/bowel control
Your symptoms are worsening rapidly without an obvious cause
