Please take a few minutes to read about our unique approach to back pain and migraines. Our approach resolves patient complaints by finding and focusing attention and treatment on the source and cause of pain rather than simply treating the symptoms brought on by pain.
Although ABC PT’s founder, Chris Ford, MPT initially developed this philosophy of care to treat patients with low back pain, we now use it to treat all forms of musculoskeletal pain and dysfunction, including migraines and headaches. This is why we consistently experience a high rate of success for these conditions.
What to Expect
When you come to the clinic, our front office personnel will request the following:
- Written PT referral/prescription (if required by insurance)
- Insurance card(s) with your ID and group number or your
social security number
- Your birth date and that of the insurance subscriber's,
(if not yourself)
- Patient Registration Information
- Co-pay (if applicable)
- Any other appropriate billing information
On your following visits, it is important that you keep us informed of any changes you may have such as change of insurance, physician or address.
Frequently Asked Questions
What should I wear?
All examinations are private. Loose-fitting, comfortable clothing is recommended. You may want to wear or bring shorts if we are treating your back, your knees or your ankles. Gowns provided upon request.
How long is each visit?
The initial evaluation, will take approximately one hour. Each follow-up visit will take approximately 45 minutes depending on your treatment plan.
What should I expect during my initial visit?
A thorough evaluation will be completed by a licensed physical therapist. This will include a patient history as well as selected musculoskeletal and neurological tests. After completion of the initial exam your therapist will discuss their findings with you. Based on your personal rehabilitation goals and the objective findings of the therapist a comprehensive treatment program will be established. This may include any of the following:
- Therapeutic exercises (including aerobic conditioning)
- Functional training in self care and home management
- Manual therapy techniques, the Leveling Technique and
soft tissue and joint mobilization
- Physical agents, mechanical/ thermal modalities
- Electrotherapeutic modalities
- Educational application of assistive, adaptive,
and protective devices and equipment
How many times a week and for how many weeks will I need to come?
The frequency and duration of your treatment program will be in part determined by your physician and therapist’s recommendations but will also be based on your goals and availability.

As a patient you have the option of seeing a physical therapist without a physician’s referral or prescription. However, you should be aware that most insurance plans will not pay for therapy without a written physical therapy referral. Call to see if your insurance requires you to have a referral. If your insurance requires a referral please contact your medical provider. Download our referral form to take to your provider.
When you are ready to schedule we will make every effort to get you scheduled to accommodate your busy life. Please have multiple times and dates you are available for appointments so we can get you scheduled quickly. After your first appointment you will be asked to schedule the rest of your visits with us.
Please be aware that your insurance plan may require a deductible and or co-payment due at time of service for each visit. To schedule your appointment, please call 253-564-2220.
Billing Policy
We are providers with the majority of insurance plans including Regence, Premera BlueCross, First Choice Health, Labor and Industries, Medicare, and Tricare. You may be eligible for insurance coverage if you have received a referral/prescription for physical therapy from your physician. As a courtesy to you, we will submit all charges to your primary and secondary insurance companies.
Unfortunately we are unable to accept Secure Horizons, United Health Care and Medicaid at this time.
Each insurance company has it’s own rules that apply for reimbursement. We try to keep abreast of the latest changes, and it is very helpful to us if our patients also understand and comply with these requirements. Please always bring your insurance card and ID with you to each visit, and notify us promptly of any changes.

Please keep in mind that many insurance plans require the patient to satisfy a deductible, co-insurance, and/or co-pay as part of coverage. For your convenience we accept Visa, Mastercard, Discover, American Express, personal checks and cash. Should payment at time of service present a hardship we will be happy to negotiate payment plans that can meet your budget.
We verify eligibility of benefits prior to your first visit so out of pocket expenses are known prior to treatment. We will also let you know if your particular insurance plan requires you to obtain a referral/ prescription and/or pre-authorization for treatment. If your injury is due to a motor vehicle accident (MVA), we will bill your Personal Injury Protection (PIP) coverage as a courtesy. However, you will continue to receive a monthly billing statement until your account is paid in full.
If you DO NOT have insurance, or we are not a provider for your plan, please let us know and we will inform you about our “Cash Pay Policy.”

We can email, mail or fax the Patient Registration Form to you. Please have them filled out before you arrive for your first appointment. If you wish for us to fax or mail the forms please request this when you call to make your initial physical therapy appointment.
Please arrive 20 minutes prior to your scheduled appointment to complete the registration paperwork, if you have not done so prior to your appointment.
For your initial appointment, you will also need your insurance information and your written referral/prescription for physical therapy.
Out of State?
We do often accommodate out of state patients. We have several options to best suit your needs. Please give us a call and we can work with you to provide the most efficient visit.
